重要事項
以下を了承の上、お申込みをお願いします。
１．ボランティアとして自分のことは自分でしっかり管理した上で、周囲と協調して積極的に活動を行います。同時にCIFTO及び受入団体のルール・指示に従い、現地の文化や社会を尊重致します。
２．申込に際しては保護者・家族に説明し、十分に同意してもらいました。従ってCIFTOに対して、私の申込・手続に対する保護者・家族や第三者から抗議は致しません。あっても、私の問題と致します。
３．手続きの代行をCIFTOに依頼し、手続きに際しての書類作成、送金、郵送等の業務をお願い致します。航空券の手配、予防接種、パスポートやビザの取得、出入国等は、私の問題と致します。
４．CIFTOの提示する参加費用の金額と規約を了承の上、お支払い致します。
５．万が一を考えて、所定の代理店を通じて海外旅行傷害保険に必ず入ります。
６．私の行動すべてその全責任は私自身にあり、CIFTOとは一切関係ありません。また、私自身の病気や怪我等の事故はもとより、天災、火災、交通事故、ストライキ、戦争、暴動、ハイジャック、盗難、詐欺、流行病、隔離等の不慮の災難に巻き込まれた場合にも、CIFTOに対して、私や保護者・家族、第三者から補償の請求は致しません。
７．私は、保護者・家族との連絡を密に保つように致します。万が一、私が保護者・家族に連絡を怠っても、それは私自身の問題であって、CIFTOとは関係ありません。
８．プログラム参加中の写真や映像を適切な範囲でCIFTOのホームページやニュースレターに掲載することに同意いたします。また、空港での待ち合わせ等のために、事前に連絡を取り合う必要があるとCIFTOが判断した場合、個人データ（氏名・メールアドレス・携帯番号）を他の参加者へ知らせることに同意いたします。
参加費について
旅行形態は「個人旅行」となっております。
参加申し込み成立後（申込書提出後）、申込者本人の都合による参加を取り消す場合には、参加費に対して以下のキャンル料が発生いたします。
１．申し込み後から出発日の30日前まで…プログラム費用の10%
２．出発日30日前から3日前まで…プログラム費用の30％
３．出発日の3日前から前日まで…プログラム費用の50％
４．出発当日…全額 出発後の途中帰国など…プログラム費用の全額
また、団体や現地NGOの都合にて事業が中止または大きな変更となる場合には、参加費については、全額返金いたします。
＊現地の事情で航空券の変更・取消料がかかっても、こちらでは一切負担できません。
＊逆に航空券（例：席が取れない）やビザなど、個人的な手続きの都合で、ツアーを変更・取消する場合も、上記の所定の費用を頂きますので、あらかじめご了承ください。

Please read important matter of below. If you agree it, please apply our study tour!
1. As a volunteer, I will manage myself firmly by myself and I will activities aggressive with emphasis on surroundings. I will respect the local culture and society according to rules and instructions of CIFTO and accepting organizations.
２. When applying, I explained to parents / family and I got fully agreed from them. Therefore, I will not protest against CIFTO from parents / family members or third parties to my application / procedure. Even if that happens, I will make it my own problem.
３. I request CIFTO on behalf of the procedure, and please do business such as document preparation, remittance, mailing etc. on procedure. Arranging air tickets, vaccinations, getting passports and visas, entering and leaving etc are my problems.
4. I pay the amount after acknowledging what terms of participation fee presented by CIFTO.
5. I definitely take out overseas travel accident insurance through the prescribed agent in the unlikely event.
6. All my actions responsibilities are entirely on my own, not CIFTO relevant at all. If I get sick or injury and involved accidents natural disaster · fire · traffic accident · strike · war · riot · hijack · theft · fraud · epidemic · source isolation, I will not request compensation to CIFTO from me, parents / family and third party against.
７. I will keep close contact with parents / family members. In the unlikely event, if I neglected to contact parents / family members, it is my own problem, not related to CIFTO.
８. I agree to post photos and videos of the program on CIFTO’s website and newsletter to the appropriate extent. In addition, if CIFTO judges that it is necessary to contact in advance in order to meet at the airport etc., I agree to notify other participants of personal data (nomination, e-mail address, mobile number)
About participation fee
The form of travel is “personal travel”.
The following cancellation fee will be charged for the participation fee when canceling participation due to the applicant’s own circumstances after the participation application is completed (after submitting the application form).
1, 30 days before the departure date after application · · · 10% of the program cost
2, 30 days before departure date to 3 days before ... 30% of program cost
3, 3 days before the departure date to the day before ... 50% of the program cost
4, Departure day or returning home after leaving etc ... full program fee
In addition, if the project is canceled or major change due to the convenience of the organization or local NGO, we will refund the participation fee in full.
＊ Even if the change and cancellation fee for the air ticket is applied due to the local circumstances, we can NOT bear anything here.
＊Please note in advance that we will charge the above-mentioned expenses when changing / canceling a tavern due to personal procedures, such as air tickets (ex: seats can not be taken) and visas.
